I, hereby grant permission to Make Work for Work and its representatives to record, photograph, and use my name, voice, image, and story for educational, promotional, or other related purposes.

I understand that my story may be used in a variety of media including but not limited to print, electronic, and online media. I acknowledge that my participation is voluntary and that I will not receive any compensation for my participation.

I further understand that my story may be edited, modified, or used in whole or in part as determined by Make Work Work for Moms]. I waive any right to inspect or approve the finished product or its use.

I represent that I have the full authority to grant the rights and permissions set forth in this release and that the use of my story and related materials will not infringe upon or violate any rights of any third parties.

I release Make Work Work for Moms, its representatives, and all others from any and all claims, demands, or causes of action that I may have against them arising out of or in connection with the use of my story or related materials.

This release shall be binding upon me, my heirs, legal representatives, and assigns.
